If possible, please include patient demographics, insurance information (copy of card if available), referring provider name and fax, reason for referral with working diagnosis, urgency level, most recent chart note, and any relevant imaging (OCT, fundus photos, widefield, FA). Complete documentation helps us schedule faster and avoids repeat testing.
